April 29, 2009: Digestive Disease Associates of Rockland, PC has been accredited by American Association for Accreditation of Ambulatory Surgery Facilities, Inc. to perform office based surgery.
At Digestive Disease Associates of Rockland we strive to develop and maintain strong relationships with our patients. We pride ourselves on using the newest, most advanced techniques in Gastroenterology. Our committed staff make every effort to ensure patient satisfaction.
We take pride in providing our patients with the comfort and convenience of five, in-office procedure suites. We perform Colonoscopies, Upper Endoscopies and Flexible Sigmoidoscopies with state of the art endoscopy equipment. Our professional clinical support staff ensures the patient of a safe and comfortable experience; Anesthesia is available for many procedures.
Our available procedure time is VERY limited. With our backlog of patients needing procedures we believe it is unfair to other patients when a procedure time goes unused. We will impose a $75 cancellation fee, if you cancel within 48 hours of your appointment.
Required Identification for Registration
The Federal Governments “Red Flag Rules for Health Care and Medical Practice” mandates that all our patients show a photo I.D. and we will match the I.D. against our patient file at every visit.
If you do not possess a photo I.D. issued by a local, state or federal government agency (Drivers license or passport), then two forms of I.D. must be presented, one of which has been issued to you by a state or federal agency (i.e. Social Security Card, utility bill or a company/school I.D.).
Additional Charges
Please be aware that you may receive separate invoices from various other providers for ancillary services rendered to you during your procedure:
- Anesthesia Services are provided by Northeastern Anesthesia Services, PC
- Pathology Technical Services are provided by Pathology Solutions or US Path
- Pathology Professional Services are provided by Drs. Rizk and Xu.
You may receive a bill for these services in addition to the charges from Digestive Disease Associates of Rockland, PC
Pathology Co Payments
Some Insurance companies require Digestive Disease Associates of Rockland, PC to collect a co pay for Pathology Service’s rendered by Drs Rizk and Xu. If your insurance company advises us that a co payment is due; we will bill you accordingly.
